Harborside Event Center 1375 Monroe St.  Ft Myers FL. 33901. Booths are $500.00 for each 10x10 booth. Each 10x10 space includes 2 8' Uncovered tables, sharps container, trash receptacle, two chairs and 3 passes. Early set up is March 3rd between 9am and 11am.

**IMPORTANT HEALTH DEPARTMENT INFO!** 
All artists must send in this form FLORIDA TATTOO ARTIST PERMIT and more info. You must send in this form along with these items

Fee of $60.00 (submit every year).
Reactivation fee of $25.00 for renewal of license after date of expiration. (If you had the license last year)
A copy of a government-issued photo identification confirming at least 18 years of age.
A copy of the certificate of training proving completion of a department approved course on blood-borne pathogens and communicable diseases with having achieved a minimum score of at least 70% on the course examination (submit for initial application only, not renewal). Please mail form and all info to this address.

You must also sign a form on site stating you have watched the FLORIDA Management of Biomedical Waste Video located at this link
